Job summary

Three Oaks Hospice is seeking a skilled social worker to provide coordinated in-home care across age groups, performing psychosocial assessments, counseling, and referrals to address patient and family needs.

Qualifications

• Bachelor's or master's degree in social work from a state-accredited program.
• Current and valid LMSW license.
• Current state social work certification.
• Good interpersonal skills and knowledge of community resources.
• Reliable transportation with valid driver's license and auto liability insurance.

Responsibilities

• Provide coordinated in-home care to patients across ages with psychosocial assessments and counseling.
• Assist with advance directives, discharge planning, family counseling, and care level changes as needed.
• Participate in IDG/IDT meetings and coordinate with the care team to plan patient care.
• Maintain HIPAA-compliant privacy and professional standards.
• Engage in ongoing education and QAPI program participation.
