Job summary

St. Croix Hospice in Brighton, MI is seeking a Registered Nurse Case Manager to provide holistic, home-based care.

You will collaborate with a care team to develop personalized plans, lead meaningful end-of-life conversations, and ensure symptom management with empathy and clinical excellence. The role covers Brighton and surrounding Lansing area; requires RN with acute care experience (hospice/home health preferred), on-call duties, and reliable transportation.

Qualifications

• Acute care nursing experience required; hospice/home health experience preferred.
• Complete initial and ongoing comprehensive assessments to determine hospice needs.
• Develop individualized care plans with patient and family; update as needs evolve.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques, medications, diet, and symptom management.
• Rotate on-call duties and ensure seamless care transitions.
• Must have reliable transportation, valid driver's license, and proof of auto insurance.

Responsibilities

• Deliver holistic, home-based patient care and pain/symptom management.
• Collaborate with care team to develop and update care plans.
