Job summary

Signify Health is seeking a Part Time Physician to perform in-home health evaluations, visiting patients in their own homes across New Jersey. Each visit lasts about 45 minutes and includes a medication/history review and a physical examination.

You may perform additional tests as ordered by the health plan. Clinicians will use an iPad and a clinical workflow; no prescribing or altering current regimens is required unless directed.

Qualifications

• Active, unrestricted medical license in one or more states.
• Multi-state licenses are preferred.
• Experience or ability in patient-facing, in-home care is valued.

Responsibilities

• Perform in-home health evaluations in patients' residences.
• Review medication histories and medical histories during visits.
• Conduct physical examinations and discuss findings with patients and health plans.
• Coordinate with health plans to order additional diagnostic tests when indicated.
• Travel locally within licensed states and conduct virtual visits as needed.
