Job summary

Precise Home Care is seeking a Home Health Aide (HHA) in Jacksonville to provide personal care under RN/therapist direction. The role serves specific clients and maintains comfort under supervision.

Requirements include CNA with 40-hour HHA training or 75 hours of HHA training, 18+ years, 6+ months supervised healthcare experience, and CPR. Valid driver’s license and strong communication skills are needed.

Qualifications

• Successful completion of a formal certification training program and/or a written skills test and competency evaluation.
• Be at least eighteen(18) years of age.
• Minimum of six(6) months work experience in a supervised setting, preferably health care facility.
• Demonstrated ability to read, write, and follow a written Plan of Care.
• Good verbal, written, and interpersonal skills.
• Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair. Good driving record.
• Current CPR certification.
• Strong communication and interpersonal skills.
• Must meet Level 2 Background requirements. As required under the directive of House Bill 531 (2025), please review the Care Provider Background Screening Clearinghouse Education and Awareness website for information on the screening process: https://info.flclearinghouse.com
• Must agree to perform drug screening on hire and randomly.

Responsibilities

• Performs simple procedures as an extension of therapy services under the direction and supervision of the therapist.
• Range of motion exercise.
• Assistance in ambulation or exercises.
• Performs personal care activities, including but not limited to bathing, shampooing, skin care/nail care, oral hygiene, shaving and dressing.
• Performs household services essential to health care at home (meal preparation/feeding, laundry, light housekeeping).
• Assists in the administration of medications ordinarily self-administered under RN direction.
• Reports changes in client condition to RN and Therapist per Care Plan.
• Documents care provided and submits required records weekly.
• Promotes personal safety and infection control; reports unsafe situations to Supervisor/Case Manager.
• Demonstrates safe use of equipment; notifies supervisor of educational needs.
• Maintains confidentiality and attends in-service programs.
• Performs other related duties as deemed necessary.
