Job summary

Herewith is seeking a compassionate Care Partner to support an in-home dialysis transition, providing one-on-one care and building a trusted routine.

Training will cover patient-specific dialysis processes and home setup, with ongoing clinical support. This role offers stable weekly hours and the chance to make a meaningful impact by helping the patient remain safe and comfortable at home.

Qualifications

• 1+ year of dialysis experience (home or clinic).
• Familiarity with home dialysis systems preferred (NxStage experience is a plus).
• Open to in-home dialysis support and willing to complete the provided training.
• Steady, supportive, and patient during transition.
• Strong communicator who collaborates with a clinical team.

Responsibilities

• Assist with dialysis treatment setup (NxStage).
• Maintain a clean, organized, and safe treatment space.
• Provide companionship and emotional support.
• Help keep treatment times on schedule.
• Provide light ADL support as needed.
• Monitor safety and report concerns to the nurse.
• Maintain simple shift notes and documentation.
