Job summary

Nova Leap Home Health is seeking a compassionate Certified Nursing Assistant (CNA) to provide direct patient care in a home health or facility setting. The CNA will assist with daily living activities, mobility, and basic hygiene while following the care plan and safety protocols.

The ideal candidate holds CNA training, has at least 1 year of relevant experience, and can reliably travel to patient locations.

Qualifications

• Must be at least 18 years old.
• Minimum 1 year of experience (preferably in home care).
• Must have access to reliable transportation.
• Must meet all requirements to maintain current certification.
• Must read and follow written or verbal instructions, and document care provided.
• Must adhere to Nova Leap Home Health policies.

Responsibilities

• Assist clients in activities of daily living and hygiene.
• Prepare meals within the patient diet.
• Assist with meals (feeding) as outlined in the plan of care.
• Assist with transfers and ambulation as needed.
• Perform light housekeeping (general cleaning, dusting, mopping).
• Meet safety needs of the client and use equipment safely.
• Complete medical records accurately and carry out assignments.
• Notify Supervisor of any acute or pertinent changes in client condition.
• Maintain client confidentiality.
• Follow and use all Universal Precaution protocols.
