Job summary

BrightCare HomeCare LLC is seeking a Certified Nursing Assistant (CNA) to provide compassionate in-home care for clients in Louisiana. Duties include assisting with ADLs, monitoring vital signs, and medication management, while maintaining confidentiality and high standards of care.

You will collaborate with families and healthcare teams to develop personalized care plans and ensure smooth care transitions in a home setting.

Qualifications

• Current certification as a CNA in Louisiana.
• High school diploma or equivalent required; healthcare-related degree preferred.
• Ability to work in a fast-paced, dynamic environment with a focus on customer service.
• Strong communication and interpersonal skills.
• Ability to maintain confidentiality and adhere to HIPAA guidelines.

Responsibilities

• Provide direct patient care at clients' homes including ADLs, vital signs, and medication management.
• Communicate effectively with clients, families, and healthcare teams to ensure seamless care transitions.
• Participate in client care conferences to develop and update personalized care plans.
• Maintain accurate and detailed records of client care and services provided.
• Adhere to BrightCare HomeCare’s policies and procedures.
