Job summary

BrightCare HomeCare is seeking a compassionate and skilled Certified Nursing Assistant Home Health Aid to provide high-quality care in clients' homes, promoting independence and comfort. You will assist with ADLs, personal care, and homemaking, monitor changes with nurses, and collaborate on individualized care plans while maintaining records and offering emotional support to clients and families.

This role requires travel to client homes in the Baton Rouge area and a Louisiana CNA

Qualifications

• Current CNA certification in Louisiana
• High school diploma or equivalent
• 3+ years of experience in home health care or related field

Responsibilities

• Provide personalized care in clients' homes including assistance with ADLs and homemaking tasks
• Monitor and report changes in clients' conditions to RNs and healthcare professionals
• Collaborate with healthcare teams to implement individualized care plans
• Maintain accurate client care records and progress
• Provide emotional support and companionship to clients and families
