Job summary

Home in the Heart Healthcare Agency seeks a Homemaker / Home Health Aide / Personal Care Assistant to provide non-medical in-home support, helping clients stay safe, comfortable, independent, and well cared for in their homes.

Responsibilities include bathing, grooming, mobility assistance, meal prep, light housekeeping, companionship, transportation, and documenting services per the agency policy, while maintaining confidentiality and HIPAA compliance.

Qualifications

• High school diploma or GED preferred.
• Previous caregiving/home health aide experience preferred.
• Able to communicate effectively with clients, families, and agency staff.
• Must complete required background screenings and employment requirements.
• Reliable transportation may be required for client visits.

Responsibilities

• Assist with bathing, grooming, dressing, and personal hygiene.
• Assist with toileting and incontinence care when needed.
• Help with transferring, mobility, and safe ambulation.
• Assist with eating and meal preparation; prepare light meals.
• Provide medication reminders per agency policy.
• Perform light housekeeping (laundry, dishes, cleaning).
• Provide companionship and emotional support.
• Assist with errands, grocery shopping, and appointments when authorized.
• Observe client condition and report changes to supervisor.
• Follow individualized care plans and document services accurately.
