Job summary

Apploi is seeking a Personal Care Provider (CNA) to deliver non-medical home care services in clients' residences. You will assist with activities of daily living, maintain a safe home environment, and provide compassionate companionship per the client’s care plan and agency policies.

Qualifications

• Must be at least 18 years of age
• Able to lift a minimum of 25 pounds
• Free from communicable diseases or health conditions
• Completion of required home care training and competency evaluation
• Ability to meet physical and mental demands

Responsibilities

• Observe and maintain a safe and secure home environment
• Report safety concerns to management
• Assist with meal preparation, light housekeeping, laundry and dishwashing
• Perform shopping and errands as outlined in the care plan
• Assist clients with appointments and community activities
• Provide companionship, conversation and social interaction
• Offer emotional reassurance and encouragement
• Promote mental stimulation through reading, writing, and engaging activities
• Assist with bathing, dressing, grooming, oral care, hair care, shaving, and nail care
• Assist with toileting, incontinence care, pericare, bedpans, urinals, and commodes
• Empty urinary collection devices and ostomy bags as outlined in the care plan
• Assist with feeding and hydration
• Provide exercise and mobility assistance as specified in the service plan
• Assist with ambulation for weight-bearing clients
• Perform transfers as outlined in the care plan
• Provide skin care of unbroken, healthy skin
• Provide medication reminders and prompting only
• Assist with limited respiratory support as needed
• Provide protective oversight and standby assistance
• Complete visit documentation with date/time/services and client signature
