Job summary

Senior Helpers - Central Long Island is seeking a compassionate Registered Nurse (RN) in Hicksville, NY to join our home health team. The RN will assess clients, develop plans of care, supervise Home Health Aides and Personal Care Aides, and ensure compliance with NYSDOH regulations in the home setting.

Ideal candidates have NYS RN license, at least 1 year of nursing experience (home care preferred), and strong assessment, communication, and documentation skills.

Qualifications

• Current New York State RN license in good standing.
• Minimum of one year nursing experience; home care experience preferred.
• Valid driver's license and reliable transportation.
• Excellent assessment, communication, and documentation skills.
• Ability to work independently and manage multiple priorities.

Responsibilities

• Conduct comprehensive initial nursing assessments and determine client eligibility for home care services.
• Develop, implement, and revise individualized Plans of Care based on client needs.
• Supervise and evaluate HHAs and PCAs to ensure safe, effective, and quality care.
• Coordinate care with physicians, Managed Long-Term Care plans, case managers, therapists, and other healthcare providers.
• Monitor client progress and promptly report changes in condition to the physician and clinical leadership.
• Maintain accurate, timely, and complete clinical documentation.
• Ensure compliance with agency policies, HIPAA, infection control, and regulatory standards.
• Participate in quality assurance, performance improvement, and staff education activities.
