Job summary

Responsibilities

• Maintain client confidentiality and privacy.
• Follow each client's care plan and provide prescribed services.
• Provide personal care including bathing, dressing, toileting, grooming and skin care.
• Assist with meals and feeding when necessary and help clients stay nourished.
• Assist with medications reminders and safe medication administration as directed.
• Perform light housekeeping and transportation to appointments as needed.
• Support client communication with family and community activities.
• Maintain accurate records of time, attendance, and tasks.
