Job summary

Yale New Haven Health at Home is seeking a Live-In Caregiver to provide compassionate, in-home care to Lifeline clients, helping them remain safely in their own homes. The caregiver will stay in the client’s home 24/7, follow the care plan, and support activities of daily living.

The role requires High School diploma or equivalent, and may require CNA/HHA certification. Candidates should be trustworthy, reliable, and able to work independently while following written directions and safety

Qualifications

• High School diploma or equivalent required.
• 2 years of personal care experience preferred if not CNA/HHA.
• HHA or CNA certification preferred.
• Ability to cook, clean, and perform homemaking tasks.

Responsibilities

• Provide personal care including bathing, dressing, toileting, grooming, skin care, mouth care, and shaving (electric razor).
• Plan and prepare meals and snacks according to dietary guidelines; assist with eating when necessary.
• Maintain client confidentiality and privacy; follow care plan and report changes to supervisor.
• Assist with medications, safe exercise/ambulation, and transfers; supervise daily activities.
• Perform light housekeeping and laundry; keep living areas clean and organized.
• Accompany client to medical appointments and assist with scheduling follow-ups.
• Assist with communication with family and community engagement.
