PossAbility, LLC is seeking a Home Care Aide to help individuals with diverse abilities live independently in Illinois. You will assist with daily tasks, provide emotional support, and advocate for client needs to improve quality of life.

Qualifications

• Minimum 1 year of Direct Care experience.
• Minimum 1 year of experience working with individuals with IDD and TBI.
• Minimum 1 year of In-Home Care experience.
• High School Diploma or GED.
• Current CPR/First Aid Certificate (Required)

Responsibilities

• Provide hands-on assistance with dressing, bathing, grooming, toileting, and ADLs/IADLs while maintaining client dignity.
• Facilitate community involvement, transport clients to appointments and social activities, and support job hunting or employment.
• Ensure access to community and build social skills with a person-centered approach; assist with safety and daily living tasks.
• Record services, health status, behaviors, and progress toward goals in accordance with agency standards.
