Job summary

The Oregon Department of Human Services is seeking a Human Services Case Manager to assess and coordinate long-term care services for Oregonians, developing person-centered plans to ensure continuity of care for older adults and people with disabilities.

You will work with the community and partner agencies to determine needs, arrange services, and monitor progress, employing equity-focused approaches and strong case-management practices in a dynamic environment.

Qualifications

• Experience reviewing case documentation, assessment results, and progress to improve outcomes.
• Experience managing a workload in a changing environment and resolving complex problems.
• Strong time management, organization, and prioritization skills.

Responsibilities

• Interview Oregonians to determine initial and ongoing eligibility for healthcare, social services, and protective needs.
• Develop detailed, person-centered case plans for clients.
• Contact clients monthly to monitor service plans and safety needs.
• Collaborate with agencies, medical providers and partners to provide benefits and information.
• Create risk mitigation plans and explain rights and options to clients.
• Inform clients of service options and recourse methods.
• Utilize forms, computer systems and available resources.
