Job summary

AccentCare seeks a hospice field visit physician to provide compassionate, patient-centered medical care for individuals with serious, advanced, or terminal illnesses in the Greater Houston area.

The role covers hospice/palliative visits, eligibility assessments, symptom management, and goals‑of‑care discussions with patients and families. Schedule is PRN, weekdays, with on-site responsibilities in Houston.

Qualifications

• Must be a licensed Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) in the state(s) of practice.
• Active DEA license.
• Valid driver's license.
• Working knowledge of hospice and palliative care, including pain and symptom management.
• Familiarity with hospice regulations and Medicare requirements.
• Strong clinical judgment and decision-making skills.
• Excellent communication with patients, families, and providers.
• Preferred hospice or hospice visit experience.

Responsibilities

• Evaluate patients for pain, symptom management, and hospice eligibility (admissions and recertifications).
• Counsel patients and families on care options and advance care planning.
• Order tests, initiate treatment, and conduct face-to-face visits as needed.
• Serve as Attending Physician when appropriate.
• Coordinate care with interdisciplinary teams and communicate with attending physicians for continuity.
• Manage patient medications.
• Participate in on-call rotation and provide 24/7 availability when assigned.
• Complete timely and compliant clinical documentation.
