Job summary

Affinity Care of Ohio is seeking a hospice aide to work under the supervision of a registered nurse, providing personal care and comfort per the patient’s plan of care. You will report observations and care performed to ensure quality and safety in the patient’s home or facility.

The role involves baths, dressing assistance, feeding support, safety monitoring, EMR documentation, and collaboration with the RN Case Manager and family members to maintain compassionate, professional care.

Qualifications

• Completion of a state approved CNA course.
• Current Healthcare Provider CPR certification.
• COVID vaccination per facility guidelines.
• High school diploma or GED.
• Ability to read and follow written instructions and document care given.
• Able to work with multip le facilities and adhere to guidelines.
• At least 18 years of age.
• Licensed driver with a valid insured vehicle.

Responsibilities

• Provide personal care including baths, grooming, and dressing.
• Assist with feeding and daily living activities.
• Maintain patient safety and use equipment properly.
• Communicate with patients and families in a respectful manner.
• Report pain, changes, or concerns to the RN Case Manager.
• Assist in ambulation and transfers as directed.
• Document observations in the EMR.
• Keep living area clean and orderly; perform light housekeeping.
• Adhere to care procedures and professional conduct.
• Participate in quality improvement activities.
