Job summary

Amara Hospice Care is seeking a dedicated Hospice Social Worker to join a compassionate interdisciplinary team serving patients and families across Metro Detroit. The role emphasizes emotional, psychosocial, spiritual, and practical support throughout the end-of-life journey and bereavement process.

This full-time position covers Metro Detroit and surrounding areas with a Monday–Friday schedule, 8:30 AM–4:30 PM, and requires strong advocacy, coordination, and compassionate communication to

Qualifications

• Licensed Social Worker (LSW) in Michigan.
• Minimum of one year of supervised experience in a medical/educational/social service setting.
• Excellent oral, written, presentation, and teaching skills.
• Strong organizational skills and ability to work independently while collaborating in a team.

Responsibilities

• Complete comprehensive psychosocial assessments for hospice patients and families.
• Educate patients and families regarding hospice benefits and available services.
• Gather and evaluate financial, social, emotional, and environmental information affecting care.
• Collaborate with patients, families, and the interdisciplinary team to develop care plans.
• Provide emotional support and crisis intervention for patients and families.
• Coordinate community resources and bereavement support.
• Maintain accurate, timely clinical documentation per agency policies.
