The Hospice Social Worker is an integral member of the interdisciplinary hospice team, providing emotional, psychosocial, spiritual, and practical support to patients and their families. This role involves assessing patient and family needs, coordinating community resources, facilitating care planning, and supporting families through the end-of-life journey and bereavement process.
This is a full-time position covering Metro Detroit and surrounding areas.
Schedule: Monday through Friday, 8:30 AM – 4:30 PM
