Job summary

Agrace in Madison, WI is seeking a Homecare Social Worker to support patient and family satisfaction through direct social work services, assessments, planning of care, and collaboration with the Interdisciplinary Team. The role requires a BSW (MSW preferred), state license, driving, and the ability to maintain confidentiality, coordinate transitions, and provide education and resources.

Qualifications

• Must hold a Bachelor’s in Social Work (BSW) with a current WI social work license.
• MSW is preferred and licensure must be active in state of employment.
• Valid driver’s license and access to reliable vehicle required.
• Strong confidentiality and documentation skills required.

Responsibilities

• Assess patient and family psychosocial, spiritual, grief and educational needs.
• Advocate for patient/family goals and coordinate care planning with the IDT.
• Coordinate transition planning with patient, family, homecare and IDT.
• Document education, interventions, and outcomes in EHR system.
• Provide education and resources to patients and families as needed.
