• Master's or Doctorate in Social Work from CSWE-accredited program.
• Active licensure as LMSW or LCSW in the state.

Responsabilidades

• Provide psychosocial support, assessment and care coordination in home settings.
• Collaborate with interdisciplinary team to connect patients with resources.
• Educate patients and families on social work interventions and plans of care.
• Perform care planning, transfers and discharge coordination.
• Identify risks and implement interventions to reduce health threats.
• Communicate status changes to healthcare team in a timely manner.
• Other duties as assigned.
