Qualifications

Associate’s Degree in Nursing is required.
Bachelor’s Degree in Nursing is preferred.
1 year home health or hospice care experience is preferred.
RN license by state is required.
CPR certification by American Heart Association is required.
Driver’s license is required.
Auto insurance is required.

Responsabilités

Determine patient eligibility for home care and hospice care.
Provide emotional, psychological, and spiritual support to patients, friends, and families.
Collaborate with multidisciplinary team to ensure optimal patient outcomes.
Assess and analyze data to determine nursing diagnoses and care plan.
Develop and revise the plan of care as patient condition warrants.
Prioritize and coordinate care delivery to improve outcomes.
Communicate changes in patient assessment to providers and team.
Promote patient independence by establishing care goals.
