Qualifications

• RN license and CPR certification required.
• Experience in hospice or home health preferred.
• Strong communication and interpersonal skills.
• Ability to triage emergent needs and collaborate with physicians.
• Proficiency with medical terminology and patient records.

Responsibilities

• Provide home-based hospice care after hours as needed.
• Monitor vitals, manage symptoms, and administer medications.
• Coordinate with an interdisciplinary team to implement the plan.
• Educate patients and families about prognosis and care goals.
• Maintain accurate patient documentation and follow agency policies.
