Job summary

Trinity Health Group in El Paso, TX is seeking a dedicated Registered Nurse for Hospice full time to join our compassionate care team. The RN will plan, organize and direct hospice services in home and community settings.

Requirements include Texas RN licensure, CPR, a valid driver's license and access to a personal vehicle; bilingual Spanish is preferred. Experience in hospice care and strong communication skills are essential for coordinating with families and the interdisciplinary team.

Qualifications

• Graduate of an accredited school of nursing with RN licensure
• 1–2 years hospice experience preferred
• Current Texas licensure, CPR and valid driver's license
• Bilingual Spanish preferred
• Personal vehicle available for transportation

Responsibilities

• Completes initial and ongoing comprehensive assessments for hospice needs
• Provides nursing care using the nursing process
• Develops and revises care plans with patient/family and care team
• Administers medications and treatments as prescribed
• Counsels patient and family in nursing needs
• Teaches self-care and health instructions; coordinates with the interdisciplinary team
• Acts as case manager for assigned caseload
• Maintains accurate clinical notes and pain/symptom records
• Communicates with physicians and coordinates orders
• Oversees equipment and coordinates services; participates in quality improvement
