Commonheart in Wisconsin is seeking a compassionate RN or LPN to join our Hospice team, delivering skilled end-of-life care to patients in their homes and local communities.

You will collaborate with an interdisciplinary team, educate families, and document changes in condition while traveling within a one-hour radius.

A valid license and driver’s license are required; this role offers a stable schedule, on-call shifts, and a full benefits package.

Qualifications

• Current RN or LPN license.
• Excellent communication and interpersonal skills.
• Hospice, home health, or long term care experience is preferred but not required.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide skilled, compassionate nursing care to hospice patients.
• Collaborate with an interdisciplinary team to develop and implement individualized care plans.
• Support patients and families through education, comfort, and advocacy.
• Document patient care and communicate changes in condition.
