Job summary

Cantex Continuing Care Network is seeking a Hospice Registered Nurse (RN) in Conroe, TX to provide skilled nursing care for hospice patients in their homes and coordinate care with the interdisciplinary team.

The RN will perform comprehensive assessments, develop and manage individualized hospice plans of care, and ensure regulatory compliance throughout the hospice care process, while working collaboratively with physicians, families, and other care professionals.

Qualifications

• Registered Nurse (RN) with state licensure
• Hospice experience preferred
• HCHB experience preferred
• Strong clinical assessment, communication, and documentation skills
• Experience working in an interdisciplinary healthcare team
• Knowledge of hospice regulations, documentation, and care planning preferred
• Reliable transportation, valid driver’s license, and current auto liability insurance
• Ability to read, write, and communicate effectively in English

Responsibilities

• Perform initial and ongoing comprehensive nursing assessments for hospice patients
• Develop, implement, and update individualized hospice plans of care to ensure quality and continuity of care
• Coordinate patient care with the Interdisciplinary Team (IDT), physicians, patients, and families
• Educate patients and families on disease processes, medications, symptom management, and end-of-life care
• Determine scope and frequency of nursing services based on patient condition and care needs
• Supervise and provide clinical direction to Hospice Aides and LVN/LPN staff
• Maintain accurate and timely clinical documentation in the electronic medical record
• Communicate changes in patient condition to physicians and interdisciplinary team members
• Ensure compliance with Medicare hospice regulations, documentation standards, and agency policies
• Participate in case conferences, IDT meetings, and care coordination activities
• Monitor assigned cases to ensure compliance with third-party payer and hospice requirements
• Promote hospice philosophy and provide compassionate, patient-centered end-of-life care
