Job summary

Amedisys, part of the Optum family, is seeking an RN Case Manager to coordinate compassionate hospice care, assess patients, and collaborate with the interdisciplinary team to develop patient-centered plans.

Responsibilities include conducting assessments, documenting progress, providing caregiver education, and participating in on-call coverage. A valid RN license and CPR certification are required.

Qualifications

• Current RN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• 1+ years of nursing experience.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Performs patient assessments and collaborates to develop and implement a care plan.
• Makes referrals to hospice disciplines as needed or documents rationale for not doing so.
• Maintains individualized patient care progress records with accuracy and timeliness.
• Records pain and symptom management changes and outcomes.
• Educates patients, families, and caregivers on care techniques.
• Keeps open communication with the clinical manager and care team.
• Performs on-call responsibilities as assigned.
• Other duties as assigned.
