Job summary

Sanctuary Hospice is seeking a compassionate Hospice Registered Nurse Case Manager (RNCM) to join our nursing team in Cincinnati. You will deliver hands-on hospice nursing care, coordinate services, and guide patients and families through the hospice journey with a focus on comfort, dignity, and connection.

You will work Monday through Friday, 8:00 am to 4:30 pm, serving the Cincinnati territory, collaborating with the interdisciplinary team and supporting patients at end of life.

Qualifications

• Active RN license required.
• Minimum one year RN experience in medical-surgical/acute care; hospice experience a plus.
• CPR certification if state requires.
• Valid driver’s license and reliable transportation.

Responsibilities

• Respond to clinical referrals with care and expertise.
• Collaborate with Clinical Director to assess eligibility for hospice services.
• Provide direct patient care focusing on comfort and quality of life.
• Deliver nursing care grounded in proven principles and compassionate practice.
• Educate patients and families to participate in personalized care plans.
• Participate in on-call duties as part of team coverage.
