Qualifications

• Must have an active RN license in the state of practice.
• Minimum 1 year experience in hospice or palliative care preferred.
• Strong clinical assessment and critical thinking skills.

Responsibilities

• Provide skilled nursing care to hospice patients at home or in facilities.
• Develop individualized care plans with the interdisciplinary team.
• Administer medication and monitor vital signs for patient comfort.
• Educate patients and families about end-of-life care and symptom management.
• Coordinate care with healthcare professionals and resources available.
