Job summary

Blue Summit Hospice and Palliative Care is seeking an RN Case Manager to deliver patient-centered end-of-life care in the Snellville community. You will coordinate comprehensive hospice plans, liaise with an interdisciplinary team, and ensure seamless communication between patients, families, and external providers.

The role requires Georgia RN licensure and at least two years of clinical nursing experience, with hospice or palliative care knowledge.

Qualifications

Current and valid RN license in Georgia.
Minimum of two years of clinical nursing experience; hospice/palliative care preferred.
Strong knowledge of hospice care principles and end-of-life care practices.
Excellent communication and interpersonal skills with patients, families, and teams.
Ability to manage multiple cases with attention to documentation accuracy.

Responsibilities

Conduct comprehensive patient assessments and develop individualized hospice care plans.
Coordinate interdisciplinary team meetings to review and update goals.
Monitor patient conditions and adjust plans to evolving needs.
Educate patients and families on disease progression, symptom management, and resources.
Serve as primary contact to facilitate communication among patients, families, and providers.
Ensure compliance with hospice regulations and quality assurance protocols.
Advocate for patient rights and dignity in end-of-life care.
Collaborate with community resources to support patients and families.
