Qualifications

• Current and valid state RN license.
• Minimum of 1 year of recent acute care experience in an institutional setting.

Responsibilities

• Completes an initial assessment of the patient and family.
• Initiates the plan of care and revises as needed.
• Regularly re-evaluates patients’ needs.
• Develops a care plan incorporating patient and family.
• Administers medications and treatments as prescribed.
• Counsels patients and families regarding care plans.
• Identifies discharge planning needs.
