Job summary

Heartland Hospice is seeking a compassionate Registered Nurse to serve patients in Wisconsin, covering Oshkosh/Berlin/Ripon or Manitowoc/Calumet regions. You will deliver skilled nursing care at patients’ homes or in related settings, guided by individualized plans of care and regular assessments.

Join a collaborative interdisciplinary team, document care, educate families, and participate in on-call duties.

Qualifications

• Current license to practice nursing in Wisconsin.

Responsibilities

• Be the primary point of contact for patients and families, guiding them through hospice.
• Deliver skilled nursing care in patients' homes or other settings per Plan of Care.
• Perform initial and ongoing assessments and adjust care as needed.
• Collaborate with IDG members to refine care plans.
• Provide education, emotional support, and bereavement guidance to families.
• Document care and communicate patient status to the care team.
• Maintain accurate clinical records and comply with infection control standards.
• Participate in on-call activities and care coordination across transitions of care.
