Job summary

Elysian Hospice, part of Cantex Continuing Care Network, seeks an experienced Hospice Registered Nurse in the Houston area. You will administer nursing care, coordinate with the interdisciplinary group, patients, and families, and ensure quality and continuity of care in the patient’s residence or care setting.

The role emphasizes comprehensive assessments, care planning, education on end-of-life care, and collaboration with the IDG to meet patient needs while maintaining strict documentation

Qualifications

• RN licensed per state requirements.
• 2 years hospice experience preferred.
• Home Care Home Base experience preferred.
• Good communication and documentation skills.
• Ability to read, write, and speak English.

Responsibilities

• Perform initial and ongoing comprehensive patient assessments and document findings.
• Coordinate care with the interdisciplinary team and family to plan and adjust care.
• Educate patients and families on disease process, end-of-life care, nutrition and self-care.
• Provide direction to Hospice Aide and LPN/LVN to ensure quality care.
• Monitor acuity and determine scope and frequency of services needed.
• Maintain infection control standards and accurate clinical documentation.
