Job summary

Elysian Hospice in Houston, TX is seeking an Admissions Registered Nurse to lead the patient intake, complete assessments, determine eligibility, and begin the plan of care in the home setting. You will work closely with patients, families, physicians, and the interdisciplinary team to ensure a smooth transition into hospice services.

Qualifications

• Graduate of an accredited Diploma, Associate, or Bachelor’s degree in Nursing.
• Current RN license (Compact preferred).
• Minimum 2 years RN clinical experience; at least 1 year of hospice experience with in-home care.
• Strong knowledge of hospice regulations, Medicare guidelines, and end-of-life care.
• Experience with clinical assessments, admissions, and care planning.
• Proficient in EMR systems (HomeCare HomeBase preferred).

Responsibilities

• Perform hospice admissions and initial patient assessments in the home setting.
• Evaluate patients using clinical judgment and LCD criteria to determine hospice eligibility.
• Develop and initiate individualized plans of care based on assessments.
• Coordinate with the Interdisciplinary Group to meet patient and family needs.
• Communicate with attending physicians, hospice physicians, and care teams.
• Educate patients and families on disease progression and hospice care expectations.
• Ensure timely and accurate documentation and required paperwork.
• Apply infection control practices per OSHA and regulatory standards.
• Participate in QAPI initiatives and performance improvement efforts.
• Join rotating on-call schedule for after-hours triage, support, and visits.
• Maintain productivity standards while delivering high-quality, patient-centered care.
