Job summary

Qualifications

• Graduate of a school accredited by the Council of Social Work Education.
• Minimum two (2) years experience in a healthcare environment.
• Hospice & Palliative care experience preferred.

Responsibilities

• Conduct timely assessments of psychological and social needs of patients.
• Provide psychosocial support and counseling.
• Participate in interdisciplinary team meetings for care planning.
• Offer pre-death bereavement support.
• Collaborate on post-death assessments.
• Deliver community education on psychosocial topics.
• Maintain documentation in compliance with standards.
