Job summary

Family Paramount Hospice LLC in Naperville, IL is seeking a part-time Registered Nurse to provide direct patient care across hospice settings. You will assess conditions and develop care plans in collaboration with physicians and the interdisciplinary team.

You will administer medications, monitor vital signs, document in Epic, educate patients and families on disease management, and support care in diverse settings including nursing homes and clinics.

Qualifications

• Provide direct patient care in hospice settings and collaborate with the care team.
• Assess patient conditions and develop appropriate care plans.
• Administer medications and treatments as prescribed by physicians.
• Document patient vital signs and medical history accurately in EMR.
• Experience in hospice or ophthalmology/ambulatory surgery a plus.
• Licensed Practical Nurses may be required to perform additional tasks as needed.

Responsibilities

• Provide direct patient care in hospice settings, hospitals and clinics.
• Assess conditions and develop care plans with the interdisciplinary team.
• Administer medications and treatments as prescribed by physicians.
• Monitor vital signs and document medical history in the EMR (Epic).
• Educate patients and families on disease management and self-care.
• Collaborate with health care professionals to ensure comprehensive care.
