Job summary

Affinity Health Management in Waterville, Maine seeks a Board Certified Music Therapist (or eligible to take the certification exam) to provide music therapy to address physical, emotional, cognitive and social needs of hospice patients.

This role involves creating individualized care plans, coordinating with the care team, educating staff and families, documenting progress and participating in quality improvement activities.

Qualifications

• Degree in music therapy approved by an accredited organization.
• Certified to practice as a music therapist in the state.
• At least 2 years of experience as a music therapist; hospice preferred.
• Current CPR certification is required.
• Understands hospice philosophy and focus on comfort.
• Licensed driver with insured vehicle.

Responsibilities

• Provide music therapy services to patients per written plan of care.
• Initiate music therapy program and train staff and families on goals.
• Prepare and submit clinical and progress summaries.
• Provide music therapy consultation to families/caregivers.
• Deliver in-service education for nursing staff as needed.
• Participate in interdisciplinary care teams.
• Engage in quality improvement and orientation activities for hospice team.
