Job summary

Affinity Hospice Management LLC in Nashville, TN seeks a Board Certified Music Therapist (or eligible) to provide music therapy addressing physical, emotional, cognitive and social needs under a written plan of care.

Responsibilities include directing therapy services, reporting patient progress, educating staff and families, and contributing to interdisciplinary teams and quality improvement; requires MT-BC or eligibility, state certification, CPR, and a valid driver’s license.

Qualifications

• Requires a baccalaureate or master's degree in music therapy approved by an accredited organization.
• Certified to practice as a music therapist within the state.
• At least 2 years of appropriate experience as a music therapist; hospice experience preferred.
• Possesses and maintains current CPR certification.
• Understands hospice philosophy; focus of care is comfort, not rehab.
• Licensed driver with insured vehicle in good working order.

Responsibilities

• Provides music therapy services to patients according to a written individualized plan of care.
• Directs music therapy treatment and determines priority needs for music therapy.
• Instructs staff and family/caregivers in music therapy goals and phases.
• Prepares and submits clinical and progress summaries.
• Provides music therapy consultation to families/caregivers.
• Gives in-service education to nursing personnel as needed.
• Participates in interdisciplinary group and quality improvement activities.
• Participates in hospice team orientation.
