The role delivers direct social work services in Home Health and/or Hospice, applying standard clinical processes, performing biopsychosocial assessments, planning care, providing crisis support, coordinating discharge planning, and documenting care for continuity across the care team.

Qualifications

• Master's Degree in Social Work from an accredited school
• Minimum 2 years preferred in healthcare
• Licensed Master Social Worker (Oregon) or Clinical Social Work Associate (Oregon) or Licensed Clinical Social Worker (Oregon)
• Basic Life Support and Driver's License required

Responsibilities

• Providing direct social work services to patients in home health/hospice.
• Developing and evaluating a plan of care with the interdisciplinary team.
• Coordinating discharge planning and ensuring timely documentation.
