Job summary

Cabrillo Hospice in California is seeking a compassionate Licensed Vocational Nurse (LVN) to provide direct patient care under an RN and follow the plan of care with the interdisciplinary team.

Your role will include documenting visits, educating patients and families, and communicating patient needs to the wider hospice team while safely using medical equipment and supporting comfort measures for terminally ill patients.

Qualifications

• Graduate of an accredited LVN/LPN program.
• Active LVN/LPN license in the state.
• Current CPR certification.

Responsibilities

• Provide hands-on care under the guidance of an RN.
• Follow the plan of care and interdisciplinary team directives.
• Document visits clearly and on time.
• Educate patients and families so they feel informed and supported.
• Communicate patient needs to the hospice team.
• Use equipment safely and responsibly.
• Support pain and symptom management and monitor responses.
• Participate in training and quality initiatives.
• Assist with other tasks as requested by the RN.
