Job summary

Ascend Hospice & Palliative Care KS seeks a Licensed Practical Nurse to join our Topeka team providing compassionate hospice care. You will coordinate with the interdisciplinary team and support patients and families in their homes or care settings.

Responsibilities include observing patient needs, documenting status, administering prescribed care, and participating in team meetings. A compassionate, collaborative approach is essential for delivering quality end-of-life care.

Qualifications

• Hold a valid and current LPN license within the state of operation.
• 1 year of clinical experience; Long Term Care/Home Care/Hospice preferred.
• Strong verbal and written communication; able to work as part of an interdisciplinary team.

Responsibilities

• Observes and documents the patient and family needs on an ongoing basis.
• Consults with the Hospice Nurse regarding patient status and symptom management.
• Delivers prescribed physical and psychological care under supervision.
• Monitors and documents patient/family input and response to care.
• Maintains continuity of patient care through ongoing communication with families and other health professionals.
• Participates in Interdisciplinary Team Meetings and related meetings.
• Maintains home care supplies for patient care management.
• Collects documentation and paperwork as requested by the Hospice Nurse.
• Assists with clinical billing as needed.
• Participates in orientation of new hospice aides.
