Parkview Health in Indiana seeks a Licensed Practical Nurse to deliver quality hospice and home care under the direction of a Registered Nurse. The LPN serves patients with dignity and respects confidentiality.
Must be a graduate of a nursing program and hold Indiana LPN license, CPR certification, and a valid driver’s license with reliable transportation. Experience in home care or related settings preferred; orientation will be provided.
The Licensed Practical Nurse has the responsibility and accountability for assuring delivery of quality nursing care for hospice and home care patients under the direction of a Registered Nurse in accordance with individual patient needs, physician orders, hospice and departmental policies. The LPN will provide hospice/palliative and home care services with respect for human dignity and the uniqueness of the patient, unrestricted by consideration of socio-economic status, personal attributes or the nature of the health problems. Maintains confidentiality of information regarding patient/caregiver, and all health care team members.
Must be a graduate of a school of nursing. Must complete hospital general orientation and department orientation (Hospice).
Must possess a current and valid license as a Licensed Practical Nurse from the Indiana State Board of Nursing. Must have valid state driver's license, reliable transportation, and proof of auto insurance. Must have current CPR certification.
Must have two years of experience in home care, oncology, critical, med/surg or nursing home.
