Job summary

Northern Arizona Hospice, part of the LHC Group within the Optum family, is seeking a Licensed Practical Nurse to deliver high-quality patient care in the patient's home.

You will perform skilled nursing tasks under RN supervision, administer medications, assist physicians, and support self-care for patients and families in a home setting. Competitive hourly pay and benefits are offered.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current CPR certification or ability to complete within 90 days of hire.
• Current driver's license and vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Performs skilled nursing services under the supervision of a staff RN daily.
• Administers prescribed medications and treatments as permitted by state regulations.
• Assists the physician and/or staff RN in performing specialized procedures as needed.
• Assists the patient with daily activities and encourages appropriate self-care daily.
• Reports to the patient's physician and other appropriate personnel regarding pertinent changes in the patient's condition as needed.
• Performs complex wound care if in-service is documented for specific procedure daily or as needed.
