H. T. PROF Group in Kansas City, MO is seeking a dedicated Hospice LPN for a full-time, Monday-Friday schedule.
You will provide skilled nursing care in home and facility settings, support symptom management, and collaborate with the interdisciplinary care team to maintain comfort and quality of life. Qualifications include graduation from an accredited LPN program, Missouri licensure, and at least one year of nursing experience; hospice experience preferred.
Qualifications
- Graduate of an accredited Licensed Practical Nurse program.
- Current LPN license valid for practice in Missouri.
- Current driver's license.
- Minimum of one year of nursing experience.
- Hospice or palliative care experience preferred.
- Reliable transportation and automobile liability insurance.
- Ability to work independently and make sound clinical decisions.
- Strong written, verbal, organizational, and interpersonal skills.
Responsibilities
- Provide direct nursing care to hospice patients in home and facility settings
- Assess patient needs and monitor symptoms, vital signs, and changes in condition
- Administer medications, treatments, and interventions as ordered
- Educate and support patients and families regarding symptom management and end-of-life care
- Communicate changes and concerns promptly to the RN and interdisciplinary care team
- Complete all required documentation accurately and on time
