Job summary

Prisma Health is seeking an LPN to administer nursing care to terminally ill hospice patients in the Hospice Of The Foothills unit, under the direction of the Registered Nurse and physician orders. Responsibilities include patient assessment, documentation, medication administration, and collaboration with the interdisciplinary team.

The role requires completion of an LPN program recognized by the SC State Board of Nursing and at least 1 year of LPN experience, with hospice experience preferred.

Qualifications

• Education: Completion of an LPN program recognized by the SC State Board of Nursing.
• Experience: One year as an LPN; hospice experience preferred.

Responsibilities

• Provide nursing care to hospice patients as delegated by the Registered Nurse.
• Monitor and document patient status and vital signs.
• Administer medications and treatments per policy.
• Communicate patient needs and changes to the RN/Provider.
• Maintain confidential patient records and documentation.
• Collaborate with the interdisciplinary team to plan and deliver care.
