Job summary

Prisma Health is seeking an LPN to administer nursing care for hospice patients during the night shift in South Carolina. You will follow physician orders and the IDG plan of care under the supervision of the Registered Nurse and ensure documentation aligns with policy.

The ideal candidate holds a SC LPN license with 1 year hospice experience preferred. Prisma Health supports compassionate care, confidentiality, and collaborative teamwork across the Hospice - Upstate facility.

Qualifications

• LPN license in SC or compact multistate license.
• 1 year hospice experience preferred.
• In lieu of 1 year hospice experience, Prisma Health Nurse Residency or comparable residency accepted.

Responsibilities

• Administers nursing care to hospice patients as per physician orders and plan of care under RN supervision.
• Documents patient status, vital signs, and labs; maintains confidentiality of records.
• Communicates patient observations to RN/Provider and team.
• Administers medications and treatments according to policy.
• Collaborates with the interdisciplinary team to plan and deliver hospice care.
• Performs other duties as assigned.
