Job summary

Continuum Care Hospice is seeking a licensed practical nurse (LPN) case manager to provide compassionate in-home and facility-based hospice care. You will work within an interdisciplinary team to implement plans of care, monitor patients, educate caregivers, and document clinical status.

Preferred experience includes hospice or home health, EMR familiarity, and end-of-life care knowledge. Must have Ohio LPN license and a valid driver's license.

Qualifications

• Current Ohio LPN license in good standing.
• Valid driver's license, reliable transportation, and proof of auto insurance.
• Strong communication and organizational skills.
• Ability to work independently while collaborating with a multidisciplinary team.

Responsibilities

• Provide direct patient care in accordance with hospice policies and physician orders.
• Monitor patient condition and report changes to the RN Case Manager and physician.
• Administer medications and treatments within scope of practice.
• Reinforce patient and caregiver education regarding disease progression, symptom management, and end-of-life care.
• Collaborate with physicians, nurses, social workers, chaplains, hospice aides, and other team members.
• Participate in care planning and interdisciplinary team meetings.
• Maintain accurate and timely clinical documentation.
• Support patients and families with compassion, dignity, and respect.
