Compassus is seeking a Hospice Licensed Practical Nurse (LPN/LVN) to provide direct, skilled nursing services under the RN Case Manager Plan of Care. You will build trusting relationships with patients and families while delivering compassionate care in home or facility settings.

This role supports caregivers with education, collaborates with the care team, and helps maintain comfort, dignity, and quality of life.

Qualifications

• Active LVN/LPN licensure required.
• Hospice experience preferred.
• Ability to educate caregivers and families.

Responsibilities

• Provide direct, skilled nursing services per Plan of Care under RN supervision.
• Build trusting, compassionate relationships with patients and families.
• Educate caregivers on care for their loved one.
