Job summary

Advanced Hospice in Irving, TX is seeking a dedicated Hospice Registered Nurse (RN) to join our home health team. You will assess patients, develop and implement care plans, administer medications, and educate families in a community-based setting.

The RN will collaborate with a Clinical Manager, coordinate patient care, participate in on-call coverage, and ensure high-quality, compassionate end-of-life care in the home.

Qualifications

• RN licensed to practice in the state.
• Graduate of NLN-accredited school of nursing.
• Must have a valid driver's license with insured vehicle.
• Minimum of two years’ nursing experience, with preference for public health or home care.
• Ability to perform physical tasks including lifting and long walking.

Responsibilities

• Complete initial patient and family assessment for home care needs.
• Regularly re-evaluate patient nursing needs.
• Initiate and revise the care plan as patient status changes.
• Administer medications and treatments as prescribed by physician.
• Coordinate patient care as Case Manager when assigned by Clinical Manager.
• Counsel patient and family in nursing and related needs.
• Prepare clinical notes and update physician at least every sixty days.
• Participate in on-call duties and ensure necessary equipment availability.
