Job summary

Swan Hospice in New Jersey is seeking a Certified Home Health Aide (CHHA) with hospice experience to join our compassionate team. You will provide in-home care to residents in private homes and facilities, supporting nurses and families with dignity and respect.

Ideal candidates hold a NJ HHA license, have at least one year of hospice experience, and ensure dependable transportation. This role emphasizes patient comfort, teamwork, and adherence to hospice philosophy.

Responsibilities

• Assists with prescribed exercise and ROM activities as taught by Hospice Nurse
• Assists with toileting, positioning and transfers for bed-bound residents/patients
• Provides ADL support: personal hygiene, grooming, skin care and nails
• Reminds or assists patient with medications in compliance with regulations
• Light housekeeping of personal unit/room including bed linen and patient laundry
• Feeds and assists with meals
• Records vital signs (temp, pulse, respiration) as supervised
• Reports changes to Hospice Nurse and documents details with date/time
